If you are setting up a new UBT Telco mobile connection or if you need to change the account that your SmartConnect payments are made from, you will need to complete a direct debit form to authorise these payments.
- Download the Direct Debit Form
- Complete the form, then print, sign and scan back to your computer
- Open a new helpdesk request
- Attach the signed copy of your form to the email, ensure you let us know which UBT account we need to change the direct debit on.
