If you are setting up a new UBT Telco mobile connection or if you need to change the account that your SmartConnect payments are made from, you will need to complete a direct debit form to authorise these payments.
- Download the Direct Debit Form
- Complete the form and save a copy to your desktop or documents
- Open a new online helpdesk request
- Attach or 'drag and drop' the saved PDF onto the new request form as shown below.
